Important information on when this form is required
Services Australia now requires all software vendors and PMS to use PRODA authentication for claims submission. Integrated online claiming software, such as Tyro Health Online usually require a Medicare Online claiming provider agreement form (HW027) to be submitted to Services Australia. To simplify registration, Tyro Health Online will pre-populate this form, including the Minor ID.
Medicare Online bulk bill or DVA claims require this form, but ECLIPSE claims may not necessarily require another form if already registered for Medicare Online - find out more about whether you need to submit another form for ECLIPSE.
This form asks for a Minor ID (also known as a Software site ID or Location ID) which links a Medicare provider number to online claiming software. Minor IDs are generated and provided by a software vendor.
A provider number can only be registered to one online claiming software vendor, so if a new form for the same provider number is submitted with a Minor ID for different software, then claiming will be enabled on the new software and disabled on others.
Instructions for completing this form
Tyro Health will pre-populate the form using information entered into Tyro Health Online, but you will need to complete additional details required by Medicare and there are some important points to note (see below).
To download your pre-populated form, login to Tyro Health Online and navigate to the 'documents' page using the sidebar. Then, select 'Medicare Online Claiming Provider Agreement'. There's also a 'do you need to submit agreement forms' tool to help you figure out if you need to complete this form.
Important points to note when completing form:
Item 1 - Name: Use your full name as registered with Medicare for the given provider number.
Item 2 - Provider number: This provider number must match that registered in Tyro Health Online.
Item 3 - PKI registration number: Leave this field blank.
Item 13 - Practice name: If you operate as a sole practitioner in your own name, use your own name.
Item 14 - Practice address: The address should match the address registered with the provider number. This is usually the practice location and not the provider’s home address.
Item 16 - Practice contact name: This could be the provider, a practice manager, owner or other administrative contact.
Item 23 - Bank account details: These details will be used for Medicare bulk bill payments to providers.
Item 24 - Type of transactions: Tick Medicare Bulk bill and Department of Veterans' Affairs claims.
Further forms may be required if you have a complex business structure. Contact us to find out what other important you may be required to send Medicare if you have providers who: operate out of multiple locations; or need to be paid into multiple bank accounts; have previously used Medicare Online claiming.

While a Medicare provide number may show as active in Tyro Health Online, it can take at least 5 business days for Medicare to process this form. They may email you once the form has been processed but we suggest you don't submit Medicare Online, DVA, or ECLIPSE transactions in the meantime as they'll be declined by Services Australia.
