Applies to: Providers using Tyro Health Online for Medicare Online Claiming.
Overview
To submit Medicare Online claims, including bulk bill and Department of Veteran’s Affairs (DVA) claims, providers may need to complete and submit the Medicare Online Claiming Provider Agreement (HW027) form.
The form links your Medicare provider number to Tyro Health’s Minor ID, enabling you to send claims securely via Tyro Health Online using PRODA authentication.
Without this form, Bulk bill and DVA claims cannot be processed.
If you process ECLIPSE claims, you may not need to complete another form if you’ve already registered for Medicare Online on Tyro Health Online. Find out more about whether you need to submit another form for ECLIPSE.
When the HW027 Form Is Required
You may need to complete a new HW027 form in the following scenarios:
1. Moving to a New Practice
If you relocate your practice, a new HW027 form must be submitted.
The Minor ID (or Location ID) on your form should reflect the new location to ensure claims are processed correctly.
2. Changing Practice Management Software
Requirements depend on how you’re set up for claiming:
If claiming through your PMS (e.g. Gensolve → Cliniko)
You must complete a new HW027 form so Services Australia can register the new software setup.If claiming via Tyro Health Online integrated with your PMS (via API key):
You don’t need to complete a new form – the existing registration covers your integration.
3. Claim Type Requirements
Bulk Bill and DVA claims: Always require the HW027 form.
Patient claims: Usually do not require the form, though this may change if Medicare updates its requirements.
How to Access and Complete the Form
Tyro Health Online simplifies the process by pre-populating your HW027 form with your practice details and Minor ID.
Step 1: Download Your Pre-Populated Form
Log in to Tyro Health Online.
Navigate to the Documents page in the sidebar.
Select “Medicare Online Claiming Provider Agreement”.
If you’re unsure whether you need the form, use the ‘Do you need to submit agreement forms?” tool on that page.
Step 2: Complete the Form
Tyro Health fills in key sections automatically, but you’ll need to complete or confirm the following fields:

3 | PKI Registration Number | Leave it blank. |
13 | Practice Name | Use your own name if you’re a sole practitioner. |
14 | Practice Address | Use the address registered with your proivder number. |
23 | Bank Account Details | Used for Medicare bulk bill payments. |
24 | Type of Transactions | Tick Medicare Bulk Bill and DVA claims. |

Step 3: Submitting the Form
Once complete and signed, send the form directly to Medicare:
Processing time: Allow at least 5 business days for Services Australia to process your form.
Your provider number may appear active in Tyro Health Online before Medicare finishes processing, but please wait to submit claims until processing is confirmed by Services Australia.
