Where to find card issue date and card issue number for manually entered private health insurance claims

When submitting a private health insurance claim, you may be prompted to enter member card details, such as a card issue date or a card issue number.

Typically, this information is printed on the members physical or digital card. Exceptions are as outlined below.

Medibank

Medibank requires Card Issue Date which, on physical cards, can be found on the front in the bottom right-hand corner. If your patient does not have a physical card, or this information is not on their physical card, then this information can be found as follows:

  • For iOS users who have set up a digital membership card, this information is available by following these steps:

    • Download the Medibank member app from the Apple App Store and follow the prompts to download your digital card to your digital wallet;

    • Open Apple Wallet on your Apple device;

    • Select the Medibank card;

    • Select the 'i' symbol for more information;

    • Scroll down to 'Card Issue' which shows the required information.

  • For all other users who cannot locate their card issue date, contact Medibank and ask for your policy start date. Your card issue date is typically 1 or 2 days after your policy start date.

Doctors' Health Fund

Doctors' health Fund requires a Card Issue Number, which:

  • For new membership cards, is located on the back of the card, in the top right-hand corner. It is the last three digits.

  • For older membership cards, is located on the back, in the bottom left-hand corner.
